Is there no value of MBBS without MD?

Exam based approach sadly have ruined MBBS. Let me explain

1 I see so many newly qualified NEET aspirants arguing on Quora that their College is best based on some parameters like new buildings or hostel. I am very sorry to say that they are wrong,

2 From the very first day you will be taught “How to pass exam”. Soon everyone will be competing for few extra marks thinking that those extra marks will translate to better clinical practice.

3 You will be taught “ How to behave in front of examiner”!!

4 The art to practice medicine hardly is taught or gets lost in front of exam.

5 The Coaching industry has become big and already installing fear that if you don’t enroll to their program you might miss the “NEXT” Bus which supposedly a clinical based exam.

6 A performance of MBBS graduate will be ultimately decided by a MCQ based exam which hardly has anything to do with real clinical scenario. NEET PG or INICET all are designed to eliminate students.

7 Let me explain a patient coming with Fever and rash can have endless possibility and to make a correct diagnosis you must be able to elicit proper history, do a examination followed by ordering relevant investigation.

In a MCQ based exam the relevant history with investigation will already be given and you just need to pick one correct choice out of four.

8 As solving a MCQ which only can have only four possibility, with all hard work like eliciting a history and clinical examination already done by the examiner, such exams can never be truly “CLINICAL”

9 If you read any answer on Quora regarding how to read Medicine for final year student, the answers will solely focus on approach to Medicine so to pass exam. This is same for all Medical colleges where in wards you will be taught the same.

10 Passing exam and practicing medicine are completely different aspects.

Now let me tell how MD is different from MBBS and becomes necessary

1 A Resident is expected to be present every morning and must arrive before the consultant. He is expected to take proper history, examine the patient and based on findings form provisional diagnosis followed by formulating a treatment plan and doing preliminary treatment.

2 All the above mentioned steps promote active learning where one is required to Solve real life problems. During the initial phase a naïve resident makes mistakes in the above mentioned steps but learns from them.

3 Other than that a Resident does active follow-up of the patient and observes the patient for years. This helps to learn about natural course of disease and how treatment modalities alter it.

4 If you compare this to MBBS, the medico comes to ward takes a history followed by the professor who teach them “ what to speak in exam”. The MBBS students hardly participates in patient care hence lack confidence regarding treating and solving real life problem. Once the class ends students never follow up the patient or check treatment sheet.

All this promote the requirement of doing MD. Nowadays I have seen answers mentioning heavy internship as a CON as they supposedly hamper your PG preparation.

A much better work culture with reduced batch size limited to 50–100 MBBS student for even big colleges is required so those students can at least get space to stand beside patient and observe while the professor teaching the examination.
