A young doctor once asked him after a long, complex brain surgery, “Sir, how do you stay so calm when the margin for error is almost zero?”

Dr. A. Marthanda Pillai replied steadily, “In neurosurgery every movement must be precise and purposeful. We prepare thoroughly, respect the anatomy, and never forget that a patient’s future depends on our care. Focus and humility keep us steady.”

That combination of surgical skill and quiet responsibility has marked his long career.

A. Marthanda Pillai (Ananthanarayanan Marthanda Pillai) graduated in medicine from Government Medical College, Thiruvananthapuram, in 1969. He joined the Kerala Medical Education Service and later completed his MS in Neurosurgery from Christian Medical College, Vellore, in 1978. He obtained the MNAMS (Neurosurgery) in 1979 and went to the United Kingdom for advanced training in microneurosurgery and neuroradiology (1980–1983), becoming a Fellow of the Royal College of Surgeons (FRCS).

He served for many years in the government health services of Kerala, where he significantly strengthened neurosurgical facilities and performed thousands of brain and spinal operations, often free of cost for poor patients. He is known for pioneering work in craniofacial surgery and upper cervical spine surgery in Kerala from the early 1980s onwards. He has published numerous scientific papers and contributed chapters to textbooks on clinical neurosciences.

In the private sector he is the Chairman and Managing Director of Ananthapuri Hospitals and Research Institute, a multi-specialty hospital in Thiruvananthapuram. He has also been actively involved with the Indian Medical Association (IMA), serving as National President and earlier as Vice-President of the Kerala State branch. He led professional protests and discussions on issues affecting the medical community, including the National Medical Commission Bill.

The Government of India awarded him the Padma Shri in 2011 for his contributions to medicine (neurosurgery). He has also received the Dr. B. C. Roy Award, the Kerala State Best Doctor Award (1998), the Dr. A. L. Sahney Award for scientific contribution, and other professional recognitions.

Even after decades of high-stakes surgery and institutional leadership, Dr. Marthanda Pillai continues to emphasise accessible care and professional ethics.

“Neurosurgery saves lives and restores function,” he has said, “but it must remain available to those who need it most, not only to those who can afford it.”

From government service that upgraded neurosurgical care in Kerala to building a major private hospital and leading the medical profession at the national level, Dr. A. Marthanda Pillai has combined surgical excellence with a strong commitment to public service and professional leadership.
