In a modest cultivator’s home in the remote village of Ghopeli (near Sipajhar) in Darrang district of Assam, Ilias Ali was born on 9 August 1955. The village had no primary school, yet the boy’s hunger for knowledge carried him forward. He completed his MBBS and later his MS in General Surgery from Gauhati Medical College, rising to become Professor of Surgery and the founder Head of the Department of Emergency Medicine at the same institution.

While practising as a skilled general and laparoscopic surgeon, Dr Ali noticed a deeper crisis unfolding in Assam’s remote char-chapori islands, tea-garden areas and Bengali Muslim settlements: families trapped in poverty by uncontrolled population growth, child marriage and the belief that family planning was against their faith. In 1993 he began a quiet, determined campaign. He travelled by foot, bicycle, motorcycle, bullock cart and country boat to the most inaccessible places, quoting verses from the Quran and Hadith to show that responsible parenthood was not forbidden. He introduced the No-Scalpel Vasectomy (NSV)—a simple, stitch-free procedure he had trained for in China—and personally motivated tens of thousands of men to adopt it.

Between 2008 and 2018 alone, more than 55,000 men underwent NSV under his influence. He trained hundreds of doctors and health workers, spoke out against polygamy and child marriage, and founded the Maleka Foundation in memory of his mother to continue charitable work. Threats and fatwas came, yet he never stopped.

One evening in a riverside village, after a long camp, a young father who had just undergone NSV looked at him and asked, “Doctor, will Allah still bless my family?”

Ilias Ali placed a hand on his shoulder and answered gently, “Allah has already blessed you with two healthy children. Now He asks you to care for them well—to give them education, food and a future. That is the greater blessing.”

In 2019 the President of India conferred upon him the Padma Shri for his pioneering work in family planning and public health. Today he continues as a senior consultant surgeon, Distinguished Professor, and tireless advocate for a healthier Assam.

From a village boy without a school to the surgeon who walked the hardest paths to bring light to families, Dr. Ilias Ali’s journey remains a luminous story of courage, compassion and unwavering commitment to social change.
