In the quiet village of Achanta Vemavaram, nestled in the fertile fields of West Godavari, a little girl named Manjula raced through the mango orchards, her braid flying, bare feet kicking up dust. She climbed trees faster than the boys, outran them in village races, and asked endless questions that made the elders chuckle.
“Why does the body heal itself sometimes and not others, Appa?” she would demand of her father, an agriculturist with weathered hands and quiet dreams.
He would smile, ruffling her hair. “One day you will find out, kanna. You will be a doctor. Our family has never had one. You will be the first.”
Manjula was the second of four siblings in a conservative home where girls rarely looked beyond the fields and the kitchen. But her father’s belief burned brighter than tradition. When she topped her class and won the Chief Minister’s scholarship after Class 10, the whole village whispered in awe. At the EAMCET, she ranked 58th—high enough to walk through the gates of Gandhi Medical College in Hyderabad.
Medicine was not an obvious path for a rural girl in the late 1980s. “People said, ‘A girl from the village becoming a doctor? She will marry and leave it,’” she later recalled with a laugh. “But Appa never doubted me.”
Tragedy struck early. In 1988, when Manjula was just 18 or 19 and newly in medical college, her father was diagnosed with cancer. Within weeks, he was gone. Her mother, only in her mid-thirties, suddenly faced the world alone with four children.
“You take care of the family,” her father had told her in those final days, his voice weak but steady. “I trust you.”
The words became her compass. Grief hardened into resolve. She studied through tears, supported her mother and siblings, and refused to let the loss define her. “Life gives challenges,” she would later say, “and standing up to them teaches you everything.”
At Gandhi Medical College she thrived—class representative, sports captain in spirit even if the fields were far behind. She loved anatomy, the precise beauty of the human body. General surgery called to her, but the operating theatres of the time were still a man’s world. Patients preferred male surgeons. Gynaecology offered another door: surgery combined with the chance to ease the quiet suffering of women who often arrived late, fearful, and resigned to pain.
She completed her MBBS, then pursued her MD in Obstetrics and Gynaecology at Osmania Medical College. Marriage to Suresh Kolli came during these years. “I did my post-graduation after marriage,” she has shared with a warm smile. “My husband and mother-in-law stood by me every step. Without family, none of this would have been possible.”
In the late 1990s, when Manjula began practising, laparoscopy—keyhole surgery—was still new in India, especially for gynaecology. The instruments were designed for larger male hands. Colleagues raised eyebrows. “A woman doing advanced laparoscopy? Are you sure?”
She was sure. She trained relentlessly in prenatal genetics, infertility, ultrasound, hysteroscopy and laparoscopy. She adapted techniques, refined them, invented new ones. One became known as the Anagani Technique—a safer way to create a neovagina for women born without one, restoring not just anatomy but dignity and possibility. She pioneered the use of a patient’s own stem cells to regenerate the endometrium when it was absent or damaged. She tackled primary amenorrhoea with innovative approaches that others hesitated to attempt.
“No case is routine for me,” she tells young doctors even now. “Each one reminds me that medicine demands humility.”
Her hands grew legendary. Over two decades she performed more than twenty thousand laparoscopic surgeries. In operating rooms from Hyderabad to Dubai (where she visits regularly as a laparoscopic surgeon), she preserved fertility when others suggested hysterectomy, removed enormous fibroids while keeping the uterus intact.
One February day in 2016 at MaxCure Hospitals, a 40-year-old woman lay on the table with a uterus crowded by fibroids. Manjula and her team worked with meticulous care. By the end they had removed eighty-four fibroids weighing four kilograms in total—the largest alone 1.07 kg—through a minimally invasive incision. Guinness World Records confirmed it: the highest number of fibroids removed from a single patient in one operation.
In 2015, the call came while she was at a national conference. The Padma Shri. India’s fourth-highest civilian honour, awarded by President Pranab Mukherjee for her distinguished service in medicine. She was elated, but more surprised for her family, who had known nothing of the nomination.
“If someone had told me five years earlier I would receive this,” she said, “I wouldn’t have believed it. It made me believe in destiny—and in responsibility. In mathematics, one plus one is two. In medicine, one plus one is three. The award came with the duty to spread that message.”
Today, as Clinical Director and Head of Obstetrics and Gynaecology at CARE Vatsalya – Women and Child Institute in Hyderabad, Padma Shri Dr Manjula Anagani continues to push boundaries. She was the first surgeon in the Asia-Pacific region to perform gynaecological robotic surgery with Medtronic’s Hugo RAS system. She trains hundreds of young gynaecologists, chairs committees, writes textbooks, and campaigns fiercely—through the NGO Pratyusha, the “No Plastic on Privates” movement against harmful sanitary products, and awareness drives for adolescent health and against unnecessary hysterectomies.
Patients still walk into her consulting room anxious. She listens first, then explains, reassures, and offers choices. “Love yourself and save yourself,” she tells them. “To achieve anything, you must be healthy first.”
From the dusty paths of a Godavari village to the gleaming theatres of international hospitals, from a father’s quiet dream to a nation’s highest recognition, Manjula Anagani’s story is not simply one of surgical brilliance. It is the story of a girl who refused to accept limits—whether of gender, geography, or grief—and who turned every challenge into a keyhole of light for the women who followed.
